Our school is participating in the Student Health Survey (SHS), a survey sponsored by the Oregon Health Authority Public Health Division and the Oregon Department of Education. The survey will gather information about health risk behaviors of students in grades 6, 8 and 11. The survey will be conducted online in the classroom and includes questions about:
• Student health and safety
• Student mental and behavioral health
• School climate and culture
Recently, a flyer was sent home that included information about the survey with an opt out form if you do not wish your child to take this survey. You need only return this form, by June 1st, if you do not wish your child to participate in the survey.
Please contact the school if you are interested in reviewing the survey document before the survey date. The survey is also available on the Oregon Health Authority (OHA) Public Health website for you to review: healthoregon.org/shs
If you have other specific questions about the survey, please feel free to call Shelly Alford at 755-2451 ext. 3011 or shellya@jewellk12.org.
